If you would like to schedule an occupational therapy evaluation, please submit  your child's PCP, primary insurance, and a brief message of your primary concern(s). Once submitted, we can begin the the intake process. We are currently placing client's onto active waitlist and look forward to meeting you as soon as session slots open!
